Post-Discharge Services

Through Post-Discharge Services, our trained Call Advisors or nurses connect with your patients 24 to 72 hours after they have been discharged from the hospital for either an inpatient or outpatient service.


Post-discharge calls serve as a communication touch point to clarify discharge instructions and remind patients to fill prescriptions and schedule follow up care. Improving communications is critical to patient care and satisfaction. Post-discharge calls can be especially helpful as some studies suggest that many patients are confused or disoriented at the time of their discharge, don’t always understand their discharge instructions and are unsure as to who to contact if problems arise.  These touch points can re-enforce your hospital’s standard of care and potentially reduce rates of readmissions and errors in medication. Readmitting a patient to the hospital after they have been discharged is a constant problem in American hospitals because it is seen by many as unnecessary and preventable with the appropriate follow-up care post-discharge.
